Referral Forms

If you are a referring dental practice and wish for the patient to be seen under the NHS, we can only accept orthodontic referrals through Vantage Rego Electronic Referral Pathway (DERS). This is a NHS regulation which affects all orthodontic practices in Kent, Surrey, and Sussex.
